FACILITIES SERVICES
CUSTOMER SATISFACTION SURVEY

About the survey application:

This survey was generated based on the needs of a local corporate entity; a global information system to collect data from their workforce concerning the facilities in which they work in an effort to improve quality of life issues throughout the organization.

The survey is completely dynamic in nature, meaning that the questions and applicable survey responses are generated based upon data residing in an enterprise database system. Possible survey responses include an input textbox, Yes/No/N/A radio buttons, and levels of approval radio buttons (Very Satisfied, Satisfied, Neither, Dissatisfied, Very Dissatisfied, and Not Applicable). The questions are input into the database as outlined by the customer along with the type of response they wish to derive.

Likewise, the data processing is conducted in the same manner. Questions are again retrieved from the database and matched against responses given. The pairing is then stored to a database table whereby analysis can be performed on the data.

Once processing and data storage is complete, a receipt is printed for the user to verify results input on the survey.
